Introduction:
Management of hard-to-heal wounds of lower extremities includes casual and local treatment. Autologous fat grafting performed within such wound is one of therapeutic approaches in this disorder. Adipose-derived stem cells present in fat may play a key role in wound healing promotion due to their proangiogenic and regenerative properties.

Objective:
In this article we have described the surgical technique of autologous fat grafting and reported cases of patients suffering from chronic wounds of various etiologies, who had undergone autologous fat grafting because of the failure of standard management.

Case report:
Significant progress in wound healing was observed in 4 cases, in 1 case the wound closed completely, however in 1 patient the effect was unsatisfactory. No adverse effects of autologous fat grafting were observed.

Conclusions:
In order to take full advantage of autologous fat’s potential, standardization of the surgical technique providing the greatest improvement in healing depending on the ulcer size and etiology is needed.
